Care options / Plan review
Plan review
Explore how progress and suitability are reviewed, when a plan may change and when a different healthcare provider may be appropriate. A care plan should remain open to questions.
Know what is being reviewed.
Follow-up care should have an explained purpose. Ask which findings or goals would be reviewed, what would change the recommendation and how a continuing plan is discussed with you. Repeating an appointment is not a substitute for understanding why it is proposed.
Before you enquire
Ask the treating clinician what information to bring to the review and how to communicate changes securely. Confirm which appointment type is being arranged and whether it is a reassessment or another form of visit.

- 01
Make a request.
Choose a broad care category and share a general question. Keep personal health information out of this form.
- 02
Confirm the visit.
The practice replies to discuss availability and the right appointment. A request does not reserve a time.
- 03
Talk it through.
Ask about the options, alternatives and practical details. An informed decision begins with clear answers.
